Plot summary
Anne Kimbrough works as a diving instructor at Hotel Elysium, a resort on a Caribbean coast. The water near a sunken shipwreck keeps claiming victims, and one of them is a student of hers. She studied marine biology, so she can tell that neither a shark nor a barracuda was responsible, though she cannot say what was. Police chief Steve Kimbrough, her estranged husband, refuses to show her the body. A tourist named Tyler Sherman keeps pestering her, and she brings him along to view the corpse at the morgue. Anne wants the resort's diving sessions called off, and her manager responds by dismissing her.

- Denied sight of his footage and shut out of editing, James Cameron broke into the editing room in Rome and cut his own version while the producers were at Cannes, but he was caught and Assonitis recut the film again.
- Drake pitched bringing Kevin McCarthy back, "all chewed up and mutilated" from the previous movie, as a scientist developing flying piranhas on an abandoned oil rig for revenge, and having Barbara Steele return so he could kill her.
- Some time before principal photography started, Assonitis fired his tentative director Miller Drake and promoted James Cameron to director, and Piranha II became Cameron's feature directorial debut.
- Cameron later employed some of the same mechanisms used to make the piranhas fly in the facehugger animatronics for Aliens.
- After the first week of shooting, quarrels broke out between James Cameron and the producers, with Assonitis arguing with most of Cameron's choices, so Cameron handled only the shooting while most decisions rested with Assonitis.
- The crew was made up essentially of Italians from Assonitis's previous films, none of whom spoke English, though some had horror and fantasy experience that partly met Cameron's requirements.
- Cameron made a deal with a distributor who bought his footage and let him re-score and re-cut the picture, and his alternative version eventually came out on home video in some regions and made a profit for the distributor.
