Plot summary
Milli Chakravarty is a physiotherapist whose Punjabi mother, Manju, hopes she will marry. A royal family in Rajasthan calls her to treat King Shekhar Singh Rathore, paralyzed from the waist down. Forty doctors have already left the job. At the palace, Queen Nirmala Devi Rathore is unimpressed by Milli's enthusiasm and clumsiness, and her easygoing manner conflicts with the household's strict rules. She meets Prince Vikram Singh Rathore, whose distant manner resembles his parents, and befriends Divya Rathore, the youngest princess, encouraging her love of cinema against the family's wish that she study business management. When Shekhar avoids the exercises Milli prescribes, she asks Vikram for help. He tells her that if she cannot stand the heat, she should leave. Frustrated, Milli accuses the family of being stubborn, rigid, and self-centered instead of helping Shekhar.

- The film's first song, "Engine Ki Seeti", samples the Rajasthani folk song "Anjun Ki Seeti", which Ikram Rajasthani specifically re-wrote for Khoobsurat.
