Plot summary
Dr. Herbert West has spent thirteen years in a maximum security prison for a murder committed by one of his reanimated corpses. Working with the little he has in the medical center, he runs basic tests on rats and isolates Nano-Plasmic Energy, a charge drawn from a living brain that, held in a small capsule, can keep a reanimated body together with his reagent. When Dr. Howard Phillips joins the prison as its new physician, West is assigned to assist him, and the young doctor's interest in the work brings him supplies. Phillips has his own reason for being there: his sister was killed by one of West's zombies when he was a boy. Journalist Laura Olney arrives to report on the prison.

Did you know?!Facts and trivia about Beyond Re-Animator (2003)
Can contain spoilers, watch the movie first.
- Having used up H. P. Lovecraft's original short story as material, Yuzna brought in writer John Penney, his collaborator on Return of the Living Dead 3, for ideas; Penney's treatment was not used, but it led to the idea of Herbert West serving a prison sentence, which became the basis of Beyond Re-Animator.
- Concept artist Richard Raaphorst, who had worked on other Fantastic Factory productions, was asked by Yuzna to create artwork for the film, but Yuzna felt none of it worked in the world of Re-Animator; some of it was later thought to have been reused in Raaphorst's own Frankenstein's Army.
- Because so much time had passed between films, and because Spain could not convincingly double for the United States, Brian Yuzna scaled the third Re-Animator picture back into a confined prison movie, with Jeffrey Combs returning as Herbert West.
