Plot summary
Seven students trained by Dr. Fuentes now provide health care in poor farming communities in the hill country, and he counts the group among his greatest accomplishments. His wife has recently died and he is near retirement. When rumors reach him that the students are lost and perhaps dead, he sets out for the outlands to search for them and finds a country different from the one he imagined, with guerrillas and soldiers in it. Padre Portillo, a priest who has lost his faith, and Domingo, a deserter from the army, join him on the road. He presses on toward the mountains, where his last student is said to live in a village.

- The film's story came from John Sayles' friends, one of them novelist Francisco Goldman, whose uncle was a doctor in Guatemala with an international health program and who later learned that most of the students he had sent to serve as rural doctors had been murdered by the government that supported the program.
- John Sayles deliberately left the country in Men with Guns unnamed and used music not tied to any single Latin American country, so viewers would not think such events could only happen in El Salvador, Guatemala or Mexico.
