Plot summary
In 1348, the plague is killing its way across England. Osmund, a novice monk, has a secret bond with Averill, a young woman who has taken shelter at his monastery. When the sickness reaches the house, he tells her to go, and she agrees to wait for him one week in a forest close by. He asks God for a sign. Ulric then arrives, an envoy of the regional bishop, needing someone to lead his band through forest and swamp to a marshland village the plague has left alone. Osmund reads the arrival as his answer and offers to guide them. Ulric's soldiers, Wolfstan, Griff, Dalywag, Mold, Ivo and Swire, hold that a necromancer rules the village and intend to take him before the bishop to be tried and killed.

In the original script the second half was entirely supernatural: Langiva was revealed to be the reincarnation of the Devil, while Osmund "was actually in Hell and Hell being in the physical place". The replacement ending was thought too dark, and producers tried to cut it from the shooting schedule, changing their minds two days before the epilogue was shot.- Despite not being credited as a writer, director Christopher Smith made very significant changes to the second half of Black Death's script, including a new ending.
- Rupert Friend and Lena Headey were attached to star in Black Death, with Famke Janssen later considered for Headey's role; after Christopher Smith was attached to direct he suggested Carice van Houten, Tim McInnerny and John Lynch for their respective roles.
- Every scene of Black Death was shot in chronological order, the order in which it appears on screen, a rare occurrence in filmmaking.
