Plot summary
Workers find a coffin while digging beside a rural cemetery. A small urn, sealed and bound to it by chains, has been underground since 1815. Monsignor Brusca has the coffin put back and the urn delivered to Michael Pierce, who studies mysticism at Rome's Museum of Ancient Art. Giselle Mares, his assistant, opens it together with Sarah Mandy, a student of art restoration and Michael's lover. Giselle cuts her finger and her blood falls on a red tunic inside. The witch Mater Lachrymarum returns, and violence sweeps Rome.

Italy's distributor Medusa Film believed Mother of Tears was too violent and asked Dario Argento to re-edit it to make it more mainstream, objecting to perverse sex in the witches' satanic gathering scenes and one cannibal killing of a major character; the Italian rating of 14 required cutting all hardcore gore, later re-instated for the DVD.- Daria Nicolodi said as early as 1984 that she and Dario Argento had finished the script for the third Three Mothers film, needing only a couple of special effects and locations, but her version was not used in whole or part for Mother of Tears.
- Dario Argento first hoped to cast a Russian model as the witch Mater Lachrymarum before giving the part to Israeli actress Moran Atias.
- An early draft of the script, set just after Inferno, had Ennio Fantastichini as a detective investigating murders at a university, with tentative casting of Chiara Caselli as a psychiatrist, Max von Sydow as a university professor and Giordano Petri as a young investigator.
- The piece at the end of Simonetti's "Mater Lachrimarum", called "Dulcis in Fondo", was performed by his heavy metal band Daemonia, and Cradle of Filth frontman Dani Filth recorded a song, Mater Lacrimarum, with Simonetti for the film's soundtrack.
