Plot summary
Alison Scott, recently promoted to an on-air job at E!, lives in Los Angeles with her sister Debbie and Debbie's family. Ben Stone is a slacker who lives off injury compensation money and works with his roommates on a website about nudity in movies. Celebrating her promotion at a club, Alison meets Ben. After heavy drinking they have a one-night stand, and a miscommunication keeps Ben from using a condom. Over breakfast the next morning they find they have nothing in common. Eight weeks later, morning sickness during an interview makes Alison take a pregnancy test, and the result shocks her. She calls Ben, who says he will support her. Alison's mother urges an abortion, but Alison decides she will keep the baby, and she worries about telling her bosses Jack and Jill.

- Anne Hathaway was cast as Alison first and walked away because Judd Apatow planned to use real footage of a woman giving birth. Seth Rogen later recalled that she did not want the crowning of the baby shown, even though it was not her own, feeling it was not her brand.
- Once Hathaway left, Jennifer Love Hewitt, Kate Bosworth and Olivia Wilde all auditioned for Alison and lost out to Katherine Heigl. Christina Aguilera was in the running too, but turned the part down because she was promoting her album Back to Basics.
- Katherine Heigl later told Vanity Fair the film was a little sexist and painted the women as shrews while the men came off as lovable. Judd Apatow did not deny the charge, but said he was shocked she used the word shrew, asking what this was, the 1600s.
- Canadian author Rebecca Eckler sued Apatow and Universal for copyright infringement, arguing the film resembled her 2005 book Knocked Up: Confessions of a Hip Mother-to-Be. Apatow replied that anyone who read the book and saw the film would see two very different stories about a common experience.
