Plot summary
Munna Bhai and his lieutenant Circuit run an extortion and kidnapping operation in Mumbai. Every year Munna's parents, Hari Prasad and Parvati, travel from their village to visit the son they believe is a doctor, and Munna turns his headquarters into a charitable hospital for the occasion, with his gang posing as staff and patients. This visit has a purpose: his parents want him married. In a park, Hari Prasad meets an old friend, Dr. Asthana, now dean of the Imperial College of Medical Sciences, and proposes a match with Asthana's daughter, Dr. Suman. When Asthana learns what Munna really does, he exposes him and humiliates his parents. Munna resolves to become a doctor, and blackmail gets him into the college, where Asthana is the dean.

- Shah Rukh Khan was cast as Munna Bhai but pulled out because of spinal problems, and the end credits thanked him for his script inputs. Before that, Rajkumar Hirani had considered Anil Kapoor for the lead during scripting.
- Because a wedding stage and an outfit for Gracy Singh would have cost several thousand rupees, Rajkumar Hirani shot the film's closing wedding stills on a real wedding stage near the sets, after a real wedding had ended.
- Sanjay Dutt, then cast as Zaheer, was switched to the lead role of Munna, a change credited with helping alter his controversial image. Vivek Oberoi had declined the part because of scheduling conflicts.
- Sunil Dutt returned to the screen after 10 years to play the father of his real-life son Sanjay Dutt, and it was the only time the two appeared together, though both had been in Reshma Aur Shera, Rocky and Kshatriya.
- The film has a similar premise to the 1998 American film Patch Adams, and producer Vidhu Vinod Chopra denied any resemblance, saying he had not watched it before release. Accusations of scene-to-scene copying later resurfaced on social media.
- Tabu and Rani Mukerji were both offered the role of Dr. Suman Asthana before Gracy Singh was cast, with Rani Mukerji turning it down over scheduling. Rajkumar Hirani had also narrated the script to Aishwarya Rai Bachchan.
- Makarand Deshpande was the first choice for the role of Circuit, which went to Arshad Warsi, and the part proved a turning point for Warsi, who had had several flops.
- The film reached silver jubilee status with a 25-week run, one of only eight Hindi films to do so since 2000, and was still playing on 300 screens across India in its 26th week.
