Plot summary
After he tried to sew his monstrous twin Belial back onto his side, Duane Bradley is separated from him and kept in a straitjacket and padded cell. Granny Ruth releases Duane and takes her family of unique individuals by bus to Peachtree County, where Doctor Hal Rockwell, her ex-husband, is to help Belial's girlfriend Eve give birth. Granny Ruth orders Duane to keep his distance from Belial, who has stopped speaking to him. When the bus stops at a drug store, Duane meets Opal, the sheriff's daughter, and asks her to help him and Belial get away; the sheriff objects to where the bus is parked, and Granny Ruth talks him into letting it leave.

- Basket Case 3 features a cover of Lloyd Price's "Personality" performed by Annie Ross, as Granny Ruth, and the Renzettes, the film's "unique individuals". While the production was trying to get the rights to the song, Lloyd and Logan Music licensed it for a dollar upon learning that Ross would be singing it.
- Frank Henenlotter wanted Basket Case 3 to have unrated gore scenes like the first film, such as Eve giving birth to Belial's babies, but Shapiro-Glickenhaus rejected this and wanted the gore toned down, forcing him to remove 11 pages of the script during filming.
- Frank Henenlotter admitted he did not have a proper idea for a Basket Case 3 story and ad-libbed much of the film on the spot. He called the production a "disaster" and apologized to co-writer Robert Martin for the quality of the scenes he delivered.

The scene in which Belial kills Brennan was originally going to use blood, but the blood was removed after Shapiro-Glickenhaus' decision to make the film less gory, leaving the effect unfinished.- Most costumes for the film's "unique individuals" were recycled from Basket Case 2, while Gabriel Bartalos created new individuals at his company Atlantic West Effects, based on designs he had been unable to make for Basket Case 2.
- After Basket Case 3, Frank Henenlotter tried to get other projects made but found that investors only wanted another Basket Case sequel, so he took a hiatus from filmmaking until his return with Bad Biology in 2008.
- Beverly Bonner called Frank Henenlotter and asked if she could appear in the film; he invited her to the set and cast her as a fast food cashier at Mighty Casey's.
